How the program works

Personal care at home, paid by Nevada Medicaid, start to finish. We handle the paperwork and the training.

  1. Apply by phone. Call 866-680-0247. We build the state application from the call and submit it with you.
  2. The assessment sets the hours. An assessor visits and scores what your loved one can't do on their own. Be honest about what they can't do. The approved hours follow the truth.
  3. Get cleared and trained. TB test, fingerprints, and free training. We schedule all of it and show you the status in the app.
  4. Care, clock in, get paid. Clock in from your phone for each visit. Every approved hour is paid by direct deposit.
Who this is for. Nevada residents on Medicaid who need help with daily living: bathing, dressing, meals, medications, getting around, or supervision. Children with disabilities and adults of any age.

Family caregivers count. Living in the same house or sharing bills does not disqualify you. The person doing the care still gets paid.
Family caregiver pay

$17 an hour, direct deposit.

Once hours are approved, the family caregiver is paid for every hour of care they provide. Training is free. Twenty hours a week is about $1,480 a month, every month, for caring for someone you already love.

Swoosh EVV · Providers

EVV from $50 a month. Billing at 5%, only when Nevada pays you.

  • Entry, $50 a month for up to 25 people: GPS, tasks, signatures, offline proving, Sandata-compliant. First month free. No extra-person charges: person 26 moves you to Starter at your next bill, and we tell you first.
  • Starter, $99 covers 25. Growth, $149 covers 50. Pro, $199 covers 200. Past the people your plan covers it is $3 a month for each extra person. 50 people on Starter is $99 plus 25 x $3 = $174 a month. Over 200 people we quote it with you.
  • A “person” is an active client or an active caregiver. Clients, caregivers and QIDPs never pay anything — only the agency does.
  • Every paid account has a small coverage fee on its bill, per account and per active caregiver. It pays for Swoosh’s own liability insurance. It is not insurance for you.
  • Sales tax, if any, is added by Stripe when you are billed. Nevada does not tax this software, so most Nevada agencies pay $0 in tax.
  • When we send your claims, we take 5% of the approved amount on the 835, pulled only after the deposit lands in your bank. Nothing on a failed claim.
  • Separate line items, no contract, no setup fee. AxisCare starts at $325 a month, HHAeXchange at $375, Alora at $295 a user.
  • Your caregivers download the app and sign themselves up. No intake on your end.
Who this is for. Nevada Medicaid home-care providers: personal care, HCBS waiver, IDD, and nursing-level care in the home. Provider types 29, 48, 58 and 83. No hospitals, no doctor appointments.

Who runs it. NV Care Solutions Inc, a Nevada provider type 30 agency. We use the same system for our own caregivers.
